Services
Structured Family Caregiving
Lifting Hearts offers a wide range of services to assist seniors and individuals by providing compassionate, personalized care that supports independent living at home.
Structured Family Caregiving (SFC) is a Medicaid-funded benefit that supports primary unpaid caregivers of persons who are generally aged, disabled, or have Alzheimer’s disease or a related dementia. The caregiver will provide supervision, personal care assistance, and homemaker services. Caregivers will be provided with education and training based on current and anticipated needs of their loved one.
PERSONAL CARE
Personal care is a high-priority aspect of health care. It involves Lifting Hearts providing these services to meet your personal needs and ensuring that you maintain a good quality of life. These services will be compelling in guaranteeing dignity, comfort, and overall well-being.

Examples: Bathing and showering, Mobility & Transfer Assistance, Feeding Assistance and Meal Prep, Medication Reminders
TRANSITIONAL CARE
When a loved one discharges from a hospital or a nursing facility, there is still more assistance needed to ensure a speedy recovery. The recovery really begins at home. Lifting Hearts can make the transition from a hospital or a nursing facility to home easier. We have a distinctive approach to care management that is created around our patient's goals while generating a safe environment for rehabilitation.

COMPANION CARE
Lifting Hearts compassionate companion care provides fellowship and protection. We aim to keep our clients engaged in social interaction, physical or mental activities and monitor their safety inside and outside of the home. While the companion will assist the client with daily living activities, such as dressing or bathing, they will spend most of their time keeping them company or making sure they’re safe in their home.

Examples: Light housekeeping, Non-medical Transportation, Assistance with hobbies, Offering conversation, friendship & emotional support, Meal planning & preparation, running errands, assistance with walks or with exercises
MEMORY CARE
Lifting Hearts Memory care offers specialized support for people living with dementia or other causes of memory issues. At Lifting Hearts we offer in-home visits, this will provide the most comfort for your loved one since it is in the comfort of their own home. Our trained and experienced caregivers take a special approach to ensure that your loved one is assisted with their activities of daily living and provided with a safe and secure environment tailored to their specific needs.

RESPITE CARE
Lifting Hearts Home Care will provide temporary relief for primary caregivers by offering short-term care and support for individuals who need assistance. These services ensure the individual receives safe, compassionate care while caregivers take time to rest, recharge, or attend to personal needs.

24/7 IN-HOME CARE
Lifting Hearts Home Care 24/7 Care provides compassionate, around-the-clock non-medical support for individuals with daily living activities. Our dedicated caregivers help ensure comfort, safety, and peace of mind day and night while allowing clients to remain in the familiarity of their own homes.

Examples: Continuous supervision, personal care assistance, mobility support, companionship, overnight care, meal preparation, medication reminders, and safety monitoring.

Private Pay
Allows clients to directly cover the cost of home care services without relying on insurance or government programs. This option provides flexibility, enabling you to choose the care that best fits your needs without restrictions on services or providers.
Long Term Care Insurance
Long-term care insurance helps cover the costs of home care services for those who need assistance with daily living activities over an extended period. If you have a long-term care policy, Lifting Hearts Home Care can work with your insurance provider to coordinate services.
CHOICE Program & GUIDE Program
These programs provide additional support options for eligible individuals who need assistance maintaining independence at home. Lifting Hearts Home Care helps families navigate available services and coordinate compassionate care based on their needs.
Workers' Compensation (OWCP)
For individuals eligible through the Office of Workers’ Compensation Programs, benefits may help cover approved home care services. Lifting Hearts Home Care works with clients to coordinate care and support their recovery, independence, and daily needs at home.​
Indiana Medicaid Waivers
For eligible individuals, Medicaid waiver programs may help cover home care services that support independence and quality of life. Lifting Hearts Home Care accepts select waiver programs to make compassionate care more accessible.
